How to Make a Referral

We’ve made the referral process as straightforward as possible so you can plan discharge with confidence. We accept referrals from hospital discharge teams, GPs, social workers, and family/self-referrals.

Our Registered Manager, personally oversees every new referral.

Get in Touch

Call or email us directly to discuss the patient’s needs. For urgent discharges, phone is always fastest.

Initial Assessment

We’ll discuss the individual’s care requirements, medical background, and home situation to establish the right support package.

Care Package Agreed

We’ll confirm the hours, visit frequency, and appropriate carer profile — including specialist skills if required.

Care Begins

A consistent, matched carer visits the individual at home. We provide handover documentation and remain available for updates.

4 hrs

Same-Day Urgent Discharge

For time-critical hospital discharge situations requiring immediate home support. Available Monday–Friday during business hours. Please call Abigail directly.

Standard
24 hrs

Standard Referral Response

All standard referrals acknowledged and assessed within 24 working hours. Care package can typically begin within 48–72 hours of referral acceptance.

Planned
48 hrs

Planned & Scheduled Care

For planned discharges or new care packages, we aim to have a care plan confirmed and a carer matched within 48 hours of your referral.

Assessment
72 hrs

Full Care Assessment

A thorough in-home assessment by a senior care coordinator, covering all needs, risks, and preferences. Completed within 72 hours where required.
